This hanger is simply the best. My used bike that I just bought was really dragging the rear cable.
I went into UC and said I need to get a rear brake hanger. Before I told them which one they asked if I wanted the Surly...Yup!
The one catch is that with the long hanger it might be to close for your saddle wire, if you have a "Y" shaped wire. You might need to get a different saddle.
This is only an issue in smaller framed bikes. Pros: barrell adjuster

I've got these on a couple of bikes, and they really help keep the cable/housing in a straighter line, which has improved my rear braking performance immensely. And, they are good looking.
My only issue is that with some grooved ferules, you cannot adjust the barrel adjuster. If you use a 5mm housing, don't use a ferule, and it works great. Swank solution Not every bike was designed with cantilevers in mind- and wish more were not. This is the best solution I've seen- looks really good. Pros: Looks good. Isn't that all cyclists care for? Cons: I switched bikes and I no longer need it. Fits right in the gap of the seat collar. Doesn't take up much space. Is long enough to allow the cable guide to fit easily. Pros: Simple. Long enough for clearance. Cons: None really. useful little item I used this to hang cable housing for cantilever brakes on a Surly crosscheck frame. It mounted easily with no clearance issues. I am now ordering a second one for another project. Pros: See above. It was easy to mount, seems plenty strong and is lightweight.
The barrel adjuster is nice to have for the rear brake, and it works well. Cons: It would be better if it were a bit longer to reduce bend (and thus friction) in the cable housing below the saddle. Maybe when the crowds realize how well this works, Surly will make them in different lengths. You could use an extender of some sort if it were crucial.
